Local anesthesia with sedation is possible, but the evidence base is smaller. A prospective cohort study compared 15 patients treated under general anesthesia with 15 treated under local anesthesia plus sedation and found no difference in overall patient satisfaction between the two anesthesia approaches. That study is useful, but it focused on satisfaction rather than a head-to-head comparison of complications or survival.

A more technical local-anesthesia study is a randomized clinical trial in which 20 patients each received 2 zygomatic implants under local anesthesia with dynamic navigation. That means 40 implants total. Osseointegration was achieved in all but one implant, yielding about 97.5% integration in that small study, and Schneiderian membrane perforation occurred in 3 of 20 cases (15%). The authors concluded that the procedure under local anesthesia was feasible and safe in that guided setting.

Some recent expert reports even suggest that local anesthesia is becoming more common in certain centers, but these are single-practice observations, not global statistics. One 2023 technical note stated that “90% of zygomatic implants are realized under local anesthesia in our practice”. That is interesting, but it should not be presented as a general global number.

Historically, zygomatic implant surgery has most often been performed under general anesthesia, especially in complex full-arch cases. However, recent studies show that in selected cases, experienced teams can successfully perform zygomatic implant surgery under local anesthesia with sedation, particularly when minimally invasive or guided techniques are used.
